Community Service Projects

VIAL OF LIFE PROJECT - INITIATED IN THE SUMMER OF 2007

    Purpose -  1. To assist community members in organizing their medical

                             information in a usable form,

                       2. To make this information available to EMS or other emergency

                            responders in the event patient can not communicate with

                            responders or simply can not remember all the information.

     Details -  1. A medical information form will be produced- to be available in

                           printed form and via this web portal,

                      2. EMS members will visit community members to assist in

                           disseminating and filling-out the forms,

                      3. Forms will placed in a folder or envelope with a bright Vial of Life

                           sticker on it,

                      4. Folder will be placed on the refrigerator,

                      5. Another sticker will be placed on the primary entrance door of the

                           residence in plan view for responders.

    Other -    1. This is to be an on-going program with periodic reassessments of

                          medical information -- every 6 months,

                     2. Forms, Folders, and Stickers will be available at the King County

                         Courthouse, Guthrie School, the 6666 Supply House, and the

                         Community Church.
